• CT residents and workers who are 18 years old and older are eligible to attend this clinic.
• If you’ve ever had a severe anaphylactic reaction to any vaccine or injectable therapy resulting in hospitalization, please bring a doctor’s note stating that it is safe for you to receive the vaccine.
• When you make an appointment through this page, your appointment will not be reflected in VAMS. If you have a VAMS account, you might receive email reminders from VAMS to make an appointment. You can ignore those reminders. We will log your vaccination into VAMS when you come to the clinic.
• If you have a fever or feel sick on the day of your appointment, please stay home.
• Wear a mask.
• Wear short sleeves for easy access to your upper arm.
• If you need to cancel your appointment, please call 203-854-7985, to open that spot up for someone else.
